Mr K has a diagnosis of Paranoid Schizophrenia and was in care for a long period as a child. Before he came to MH Reablement Mr K had frequent re:admissions to hospital, his risks included social isolation and self:neglect. Our Independent Living Assessor/ Occupational Therapy Care Manager (OTCM) of the MH Reablement Team worked closely with MR K to support him with his activities of daily living and he became independent in managing laundry, personal care tasks and completing meal preparation independently. We improved his health and nutrition, reduced hospital admissions and he met his re:ablement goals and continues to flourish independently within our community. The Role: As a Team Manager within the Joint Homelessness Team (JHT), you can make your own powerful contribution by helping us support some of the most vulnerable and disadvantaged people in society. Working with rough sleepers across Westminster, the JHT is an inspirational community mental health service that collaborates with colleagues from the housing department and health workers. Taking the lead of this friendly and collaborative team, you will therefore see to it that a high:quality service is being offered to people faced with homelessness, who are also struggling with mental illness. This is an exciting opportunity to lead a team that includes multidisciplinary professionals including Social Workers, Nurses, Doctors and Administrative Support colleagues. As such, you will ensure we are delivering a unified service that is both flexible and creative, and is tailored to the individual needs of each service user. Utilising an extensive network of organisations who are able to underpin our work (such as housing providers and inpatient mental health services), you will put safeguarding at the heart of every decision, and ensure every action we take is within the relevant legislative framework. Crucially, you will be an inspirational leader to the team, ensuring workloads are manageable and that the relevant learning opportunities and supervision are provided to more junior colleagues and apprentices : and in the process, you will see to it that we are growing a pipeline of talent, ready to meet the future needs of Westminster's community. Essential Qualifications: You must be a registered Social Worker with Social Work England. Please refer to the Job Description for more information. About You: As a registered Social Worker with a background in working in health or social care mental health services within the community, you have an in:depth knowledge of working with socially excluded groups, with a particular emphasis on those with complex needs and those who have a history of homelessness.
