We are working closely alongside a Local Authority in Islington to assist with the appointment of a Case Manager , on a 3 -month contract , likely to be extended at clients discretion. Rate of Pay: £20.59 - £26.96 per hour Responsibilities: The post holder will exemplify best practice in working with adults in strengths based and person centred way, connecting people with informal, community based support wherever possible. You and other team members will be fully committed to working in partnership with residents, carers, community based organisations and other key professionals involved in enabling people to be as independent as they possibly can. Deliver strengths based quality conversations, signposting and early intervention and prevention which enable our residents to lead healthy independent lives outside of formal adult social care wherever possible. Qualifications : Qualification credit Framework level 3 (QCF) in health and social care or will to train to achieve this. Experience of working with adults in Social Care/Health/Voluntary Community Sector setting. A working knowledge of relevant legislation and guidance relating to the provision of services for adult client groups. Please note : You should be available to work immediately or at a short notice.
