This is a long term case team.We are looking for an Adult's Social Worker to join the duty learning disabilities team.THIS POST REQUIRES A SOCIAL WORK QUALIFICATION OF A MINIMUM 2 YEAR POST EXPERIENCE. About the teamThis team works with individuals that have learning disabilities and/ or autism. This is a long term case team. Responsibilities within this team will include seeking funding from the council in relation to their care package, work with other services such as OT’s and support workers to put together care plans and to carry out necessary legislative assessments. This team is a highly successful team with supportive management within the service. This team prioritises the importance of promoting independence of decision making for individuals. About youYou will be a committed social worker that practices person centred approaches and values the importance of equality and diversity within adult social care. Experience of working with learning disabilities, transitions 0-25 and/ or complex care is well sought after within this team. You must have a qualified social work degree with a minimum of 1 year post ASYE.
