I am on the hunt for an experienced adults social worker - hospital discharge experience is a must for this role with West Lothian Council based from St John's Hospital.Responsibilities:In this role, you will be responsible for: Hospital Discharge: Coordinating and facilitating safe and timely discharges from hospitals, ensuring all necessary support is in place for a smooth transition back to the community or to appropriate care settingsTriage/Duty Services: Responding to urgent cases, assessing needs, and making informed decisions to provide the right level of support at the right timeD2A Reviews: Conducting Discharge to Assess reviews, assessing individuals in their own homes or care settings to determine ongoing care and support needsCHC MDTs: Participating in Continuing Healthcare Multi-Disciplinary Team meetings to assess and review eligibility for CHC funding and to ensure appropriate care plans are in placeHospital Avoidance: Working proactively to prevent unnecessary hospital admissions by providing timely interventions and support in the community Requirements: A degree in Social Work and registration with SSSCExperience in adult social care, particularly in hospital discharge, triage/duty, or similar rolesAccess to your own carEnhanced PVG If you are interested, please call Oscar on 01772 XXXXXX or email me your CV - XXXX@servicecare.org.uk
